For relatives, family friends, neighbors, coaches, clergy, and other individuals with firsthand knowledge of the child or family. Your observations may help provide additional context regarding the child’s relationships, daily functioning, home environment, and overall well-being as part of a court-appointed Social Investigation, Guardian ad Litem appointment, or Parenting Coordination matter.
Teachers and School Staff
For teachers, school counselors, administrators, school psychologists, and other educational professionals who work directly with the child. Information regarding the child’s academic performance, behavior, peer relationships, attendance, and educational needs may assist in understanding the child’s functioning within the school environment as part of a court-appointed Social Investigation, Guardian ad Litem appointment, or Parenting Coordination matter.
Mental Health Professionals
For mental health professionals who have provided evaluation, assessment, consultation, or therapeutic services to the child or family. Professional observations, clinical impressions, and treatment-related information may assist in understanding the child’s emotional, behavioral, and developmental functioning as part of a court-appointed Social Investigation, Guardian ad Litem appointment, or Parenting Coordination matter, subject to applicable legal authorizations and confidentiality requirements.

Parenting Coordination Issue Submission
This form is intended to notify the Parenting Coordinator of a specific dispute or implementation issue related to the Parenting Plan or court order. Please complete one form for each issue. Before submitting this form, make a good-faith effort to communicate directly with the other parent and resolve the matter if it is safe and appropriate to do so.
